การศึกษาวิจัยครั้งนี้มีวัตถุประสงค์เพื่อค้นคว้ารูปแบบการลด ละ เลิก การบริโภคเครื่องดื่มแอลกอฮอล์ในเยาวชนอายุ 15 – 19 ปี โดยอาศัยการศึกษาแนวคิดทฤษฎีและการทบทวนวรรณกรรมที่เกี่ยวข้องเพื่อหาพฤติกรรมและสาสเหตุการบริโภคเครื่องดื่มแอลกอฮอล์และนโยบายสาธารณะจากความคิดเห็นของเยาวชนโดยใช้วิธีวิจัยแบบผสมผสานและมีการทวนสอบให้สอดคล้องกับบริบทของสภาพการณ์ที่เป็นจริงจากการสัมภาษณ์เชิงลึกเยาวชนอายุ15 – 19 ปี จำนวน 25 ท่านใน 5 จังหวัด การสังเกตุแบบมีส่วนร่วมในการใช้บริการในสถานประกอบการของเยาวชน ในพื้นที่ 5 จังหวัดได้แก่จังหวัดชลบุรี ลพบุรี ขอนแก่น เชียงใหม่ สงขลา และการเก็บข้อมูลเพื่อวิเคราะห์ทางสถิติ จากกลุ่มตัวอย่างที่เป็นเยาวชนอายุ 15 – 19 ปี 450 ชุด ในพื้นที่ 5 จังหวัดได้แก่จังหวัดชลบุรี ลพบุรี ขอนแก่น เชียงใหม่ สงขลา โดยใช้ SPSS ในการวิเคราะห์ข้อมูลเพื่อหาค่าเฉลี่ย และนำข้อมูลที่ได้ไปให้ผู้ที่เกี่ยวข้องในการกำหนดนโยบายทำการอภิปรายและแสดงความคิดเห็นต่อนโยบายที่มาจากความคิดเห็นของประชาชน
ผลการวิจัยปรากฏว่า เยาวชนคิดว่าตนเองไม่ได้ดื่มในปริมาณที่อันตราย เลือกดื่มเบียร์หรือเครื่องดื่มแอลกอฮอล์ที่มีราคาไม่สูง หรือผู้จำหน่ายจัดกิจกรรมส่งเสริมการขาย โดยสถานที่ที่นิยมดื่มเป็นที่แรกคือหอพักก่อนจะไปใช้บริการในสถานบันเทิง และสาเหตุการดื่มคือเรื่องของสังคมและการคบเพื่อนโดยสิ่งแวดล้อมก็เป็นปัจจัยต่อการบริโภคเพราะสิ่งแวดล้อมที่มีผู้ดื่มแอลกอฮอล์ส่งผลให้เยาวชนเกิดความรู้สึกคุ้นเคย อยากลองไม่อยากที่จะแตกต่างจากสมาชิกในสังคมตนเอง ซึ่งนโยบายและมาตรการทางกฎหมายที่เยาวชนเกรงกลัวคือพระราชบัญญัติการจราจรทางบก พ.ศ. 2522 ที่กำหนดว่าผู้ขับขี่ซึ่งมีอายุต่า กว่ายี่สิบปี บริบูรณ์ ห้ามขับขี่ในกรณีมีปริมาณแอลกอฮอล์ในเลือดเกิน 20 mg% เนื่องจากเห็นการปฏิบัติของเจ้าหน้าที่ตามมาตรการนี้เสมอและไม่ต้องการให้ครอบครัวเดือดร้อนดำเนินการ กิจกรรมที่มีส่วนต่อการลดละเลิกในความคิดเห็นของเยาวชนคือการส่งเสริมให้เยาวชนสามารถเข้าถึงการณรงค์และการใช้เวลาและโอกาสที่สร้างสรรค์ทางสังคม โดยกำหนดบทบาทให้เยาวชนมีส่วนร่วม โดยรูปแบบการลดละเลิกตามผลการศึกษา พบว่า การลดประกอบด้วยการจำหน่าย การขายและการสร้างค่านิยมให้เรื่องของการดื่มเครื่องดื่มแอลกอฮอล์ไม่เป็นเรื่องปกติ รวมไปถึงผลที่จะได้รับจากรูปแบบหากละเมิดรูปแบบจากการลดในมิติของการควบคุม รูปแบบการละ ประกอบด้วยนโยบายที่เกี่ยวข้องกับการป้องกันการเพิ่มขึ้นของนักดื่มหน้าใหม่ที่เป็นเยาวชนด้วยการสร้างทักษะชีวิต เสริมภูมิคุ้มกันด้วยการสร้างสังคมปลอดเครื่องดื่มแอลกอฮอล์ รูปแบบการเลิก ประกอบด้วยนโยบายที่เกี่ยวข้องกับการเข้าถึงกลุ่มเสี่ยงและช่วยเหลือด้วยการบำบัดรักษาทั้งทางจิตสังคม และการรักษาทางการแพทย์

วัคซีนใจ คือ การส่งเสริมให้คนมีความสามารถที่จะปรับตัว ปรับใจกับเหตุการณ์วิกฤตของชีวิต มีพลังใจในการดำเนินชีวิต และกลับคืนสู่ภาวะปกติหลังจากที่พบกับเหตุการณ์วิกฤตหรือสถานการณ์ที่ทำให้เกิดความยากลำบากในชีวิตได้ แบ่งเป็น 3 ระดับ 1. วัคซีนใจในบุคคล ด้วยพลัง อึด ควบคุมอารมณ์ คิดเชิงบวก ฮึด ความหวังในชีวิตสร้างกำลังใจ สู้ เป็นพลังเอาชนะอุปสรรคด้วยการปรับและยืดหยุ่น 2. วัคซีนใจในครอบครัว 3 พลัง พลังบวกมองทางออกในทุกปัญหา พลังยืดหยุ่น ปรับเปลี่ยนบทบาทแบ่งเบา พลังร่วมมือให้ครอบครัวเป็นน้ำหนึ่งใจเดียว 3. วัคซีนใจในชุมชน 4 สร้าง ความรู้สึกปลอดภัย สงบ ความหวังเข้าใจและให้โอกาส โดยใช้ศักยภาพและสายสัมพันธ์ที่แน่นแฟ้นและมีอยู่ในชุมชน โดยเฉพาะวิกฤตการระบาดของโควิด 19 วัคซีนใจสามารถนํามาใช้ได้ทั้ง 2 สถานการณ์ 1. ในสถานการณ์ปกติ 2. ในสถานการณ์วิกฤต นําหลักการของการเสริมสร้างวัคซีนใจมาใช้ เพื่อให้สามารถดูแลจิตใจของตนเองและผู้อื่นได้ หากเกิดเหตุการณ์วิกฤตขึ้นในอนาคต วัคซีนใจจะเป็นเครื่องมือสําคัญที่ช่วยให้คนที่กําลังเผชิญกับวิกฤต สามารถสร้างพลังใจและมองหาทางออก

ภาพข่าวตามสื่อต่างๆ ที่นำเสนอเหตุการณ์รุนแรงสะเทือนขวัญที่เกิดขึ้นในครอบครัวหรือสังคม ผู้ก่อเหตุส่วนใหญ่เป็นบุคคลปกติทั่วไปที่ไม่ได้มีปัญหาด้านจิตเวช แต่มีส่วนหนึ่งที่ผู้ก่อเหตุมีประวัติเป็นผู้ป่วยจิตเวชที่เคยเข้ารักษาในโรงพยาบาลมาก่อน เมื่อกลับไปอยู่กับครอบครัวหรือชุมชนแล้วมีปัญหาขาดการรักษาต่อเนื่อง ขาดยา ไม่ยอมรับการเจ็บป่วย หรือใช้สุรา/สารเสพติด จึงทำให้อาการกำเริบ มีพฤติกรรมก้าวร้าว และอาจก่อความรุนแรงทั้งต่อตนเองผู้อื่น หรือทรัพย์สิน กรมสุขภาพจิต เห็นความสำคัญและกำหนดนโยบายการแก้ไขปัญหาความรุนแรงในผู้ป่วยจิตเวชโดยให้หน่วยบริการจิตเวชทุกแห่งมีการดำเนินงานดูแลผู้ป่วยจิตเวชที่มีความเสี่ยงสูงต่อการก่อความรุนแรง (Serious Mental Illness with High Risk to Violence) หรือเรียกว่า ผู้ป่วย SMI-V มาอย่างต่อเนื่อง ตั้งแต่ปีงบประมาณ 2559 มีการคัดกรอง ลงทะเบียน ให้การบำบัดรักษาที่เฉพาะเจาะจง และนำระบบการจัดการรายกรณี (case management)มาใช้ในการดูแลผู้ป่วย SMI-V ที่มีปัญหายุ่งยาก ซับซ้อน เน้นความร่วมมือระหว่างสหวิชาชีพในการดูแลผู้ป่วยรวมถึงการติดตามดูแลอย่างต่อเนื่อง ตามคู่มือการดูแลผู้ป่วยจิตเวชที่มีความเสี่ยงสูงต่อการก่อความรุนแรง สำหรับสถาบัน/โรงพยาบาล สังกัดกรมสุขภาพจิต ซึ่งบุคลากร/ผู้รับผิดชอบการดูแลผู้ป่วย SMI-V ได้ใช้เป็นแนวปฏิบัติมาตั้งแต่ปี 2561 และในปี 2563 ได้มีการทบทวนคู่มือดังกล่าวโดยมีการปรับนิยาม รายละเอียดของเกณฑ์ กระบวนการดำเนินงานให้มีความชัดเจนมากขึ้น ส่วนเนื้อหาที่ยังคงเหมือนเดิม เช่น ตัวอย่างการดูแลผู้ป่วย SMI-V โดยการจัดการรายกรณี(case management) และเนื่องจากคู่มือดังกล่าวเน้นกระบวนการดูแลรักษาเชิงระบบ (System intervention)จึงได้ปรับชื่อใหม่เป็น “คู่มือระบบการดูแลผู้ป่วยจิตเวชที่มีความเสี่ยงสูงต่อการก่อความรุนแรง สำหรับสถาบัน/โรงพยาบาล สังกัดกรมสุขภาพจิต”
กรมสุขภาพจิต โดยกองบริหารระบบบริการสุขภาพจิต หวังเป็นอย่างยิ่งว่า “คู่มือระบบการดูแลผู้ป่วยจิตเวชที่มีความเสี่ยงสูงต่อการก่อความรุนแรง สำหรับสถาบัน/โรงพยาบาล สังกัดกรมสุขภาพจิต” เล่มนี้ จะเป็นประโยชน์ต่อผู้ปฏิบัติงานในการจัดบริการดูแลผู้ป่วย SMI-V ให้เป็นแนวทางเดียวกัน เกิดคุณภาพและส่งผลลัพธ์ที่ดีต่อการให้บริการผู้ป่วยจิตเวช ครอบครัว ชุมชนและสังคมต่อไป
