ตามที่กระทรวงสาธารณสุขได้กำหนดนโยบาย 13 ด้าน เพื่อขับเคลื่อนด้านการบริการทางการแพทย์ ให้ครอบคลุม พร้อมส่งเสริมงานสุขภาพจิต ลดช่องว่างการเข้าถึงบริการจิตเวชและยาเสพติด เชื่อมโยงการทํางานทุกภาคส่วน ซึ่งหนึ่งในนโยบายระยะสั้นและเร่งด่วน (Quick Win) คือ ด้านสุขภาพจิต และยาเสพติดโดยกำหนดให้มีการจัดตั้งมินิธัญญารักษ์ทุกจังหวัด การเปิดหอผู้ป่วยในจิตเวชและยาเสพติด ในโรงพยาบาลศูนย์/ โรงพยาบาลทั่วไป รวมทั้งมีกลุ่มงานสุขภาพจิตและยาเสพติดในโรงพยาบาลชุมชน ทั้งนี้เพื่อเพิ่มการเข้าถึงบริการของผู้ป่วยยาเสพติด โดยสามารถเข้ารับการรักษาใกล้บ้าน ให้ได้รับการดูแล บำบัดรักษาอย่างครบวงจร ซึ่งจะช่วยลดความเสี่ยงในการก่อความรุนแรงในสังคม รวมทั้งเสริม ศักยภาพให้แก่บุคลากรที่ให้บริการบำบัดรักษาผู้ป่วยยาเสพติดและจิตเวชยาเสพติดในมินิธัญญารักษ์ในทุกจังหวัดให้เป็นไปอย่างมีประสิทธิภาพ
กรมสุขภาพจิตจึงร่วมกับผู้เชี่ยวชาญ ผู้ปฏิบัติงาน ตลอดจนผู้รับผิดชอบงานด้านยาเสพติด จากหน่วยงานภาคีเครือข่ายในกระทรวงสาธารณสุข ได้แก่สถาบันบำบัดรักษาและฟื้นฟูผู้ติดยาเสพติด แห่งชาติบรมราชชนนี โรงพยาบาลธัญญารักษ์ขอนแก่น กรมการแพทย์ โรงพยาบาลในสังกัดสำนักงาน ปลัดกระทรวงสาธารณสุข และกองบริหารการสาธารณสุข จัดทำคู่มือการดูแลผู้ป่วยจิตเวชยาเสพติด ระยะกลาง (Intermediate Care) ขึ้น เพื่อสนับสนุนการดำเนินงานตามนโยบาย และเพื่อให้บุคลากรสาธารณสุขที่ปฏิบัติงานใน มินิธัญญารักษ์มีแนวทางในการดูแลผู้ป่วยยาเสพติดและจิตเวชยาเสพติดอย่างมีคุณภาพ

วัสดุและวิธีการ : การวิจัยและพัฒนา ดำเนินการ 3 ระยะ ดังนี้ ระยะที่ 1 การวิเคราะห์ความต้องการ/ ความจำเป็นในการพัฒนา ระยะที่ 2 การวิจัยและพัฒนาเพื่อพัฒนาต้นแบบโปรแกรมและตรวจสอบคุณภาพจากความตรงเชิงเนื้อหา ระยะที่ 3 การวิจัยกึ่งทดลองแบบสองกลุ่มวัดก่อน หลังและติดตามเพื่อประเมินและเปรียบเทียบผลของโปรแกรมฯในผู้ป่วยโรคจิตเภท วิเคราะห์ข้อมูลด้วยสถิติพรรณนา สถิติ Repeated Measure ANOVA
ผล : ระยะที่ 1 การสัมภาษณ์ผู้มีส่วนเสียพบว่า ผู้ให้บริการต้องการให้ปรับเพิ่มกิจกรรมครอบคลุมถึงญาติ ระยะที่ 2 การพัฒนาต้นแบบโปรแกรมส่งเสริมความร่วมมือในการรักษาด้วยยาของผู้ป่วยโรคจิตเภทประกอบด้วยกิจกรรมกลุ่ม ๆละ 60 นาที จำนวน 6 กิจกรรม ระยะที่ 3 การศึกษาผลกลุ่มตัวอย่าง 100 คน (ผู้ป่วย 50 คน ญาติ 50 คน) แบ่งเป็นกลุ่มทดลองและควบคุม กลุ่มละ 25 คน พบว่า เมื่อเวลาผ่านไป ในกลุ่มทดลอง พบว่า ผลคะแนนพฤติกรรมร่วมมือในการรักษาด้วยยา ในระยะหลังการทดลองทันทีและระยะติดตามผล 1 เดือนเพิ่มขึ้นมากกว่ากลุ่มควบคุมอย่างมีนัยสำคัญทางสถิติ (p<0.001)
สรุป : โปรแกรมส่งเสริมพฤติกรรมความร่วมมือในการรักษาด้วยยา ช่วยส่งเสริมผู้ป่วยให้มีพฤติกรรมความร่วมมือในการรักษาด้วยยา
