จากข้อมูลปัญหาพฤติกรรม-อารมณ์ที่สำคัญในเด็กวัยเรียนพบว่า พฤติกรรมรังแกกัน (bullying) ในประเทศไทยมีสัดส่วนที่ค่อนข้างสูงและเป็นอันดับ 2 ของโลก ซึ่งผลกระทบที่เกิดจากการรังแกกัน (bullying) ส่งผลมากมายต่อร่างกาย จิตใจและการดำเนินชีวิตของทั้งนักเรียนที่เป็นผู้รังแกและเหยื่อ หากผู้มีส่วนเกี่ยวข้องปล่อยทิ้งไว้หรือมองว่าเป็นเพียงการแกล้งกันตามธรรมดาของเด็ก อาจนำไปสู่การก่อปัญหาอาชญากรรมหรือการทำร้ายผู้อื่นในเด็กที่เป็นผู้รังแก ส่วนนักเรียนที่เป็นเหยื่อ ได้รับบาดเจ็บ เกิดปัญหาสุขภาพจิต อาจรู้สึกอับอาย ซึมเศร้า วิตกกังวล เก็บกด ก้าวร้าว
ฆ่าตัวตาย นอกจากนี้ บุคคลที่อยู่รอบข้างเด็ก เช่น ครู เพื่อน พ่อแม่ผู้ปกครอง โรงเรียน สังคมก็อาจได้รับผลกระทบจากพฤติกรรมอันเป็นผลกระทบจากการรังแกของเด็กด้วย
โปรแกรมเล่มนี้พัฒนาขึ้นโดยมีวัตถุประสงค์ใน 2 กลุ่มคือ 1) ผู้ใช้โปรแกรม และ 2) กลุ่มเป้าหมายโดยในกลุ่มผู้ใช้โปรแกรม ประกอบด้วย ครูประจำชั้น ครูที่ดูแลระบบดูแลช่วยเหลือนักเรียน (ครูแนะแนว/ครูที่ปรึกษา) มุ่งหวังให้ครูเกิดความตระหนัก เห็นความสำาคัญในบทบาทของตนที่จะช่วยเฝ้าระวังพฤติกรรมรังแกกันในเด็กนักเรียนประถมศึกษาที่ 1-3 มีความรู้ที่ถูกต้อง และมีทักษะในการจัดการ แก้ไข เกี่ยวกับพฤติกรรมรังแกกัน รวมทั้งสามารถพัฒนาทักษะการควบคุมตนเองในเด็กกลุ่มเป้าหมาย ส่วนวัตถุประสงค์ที่คาดหวังให้เกิดกับนักเรียนประถมศึกษาที่ 1-3 คือเด็กมีทักษะในการควบคุมตนเอง (self-control) ตลอดจนสามารถมีทักษะในการป้องกันตนเองจากการถูกรังแกและมีทักษะในการรับมือเมื่อถูกรังแกได้

โรคออทิสติกเป็นโรคพัฒนาการทางสมองที่พบเจอได้บ่อยในปัจจุบัน แม้ว่าจะเป็นโรคที่มีความผิดปกติตั้งแต่กำเนิด แต่กว่าพ่อแม่จะสังเกตรู้มักจะเป็นช่วงวัย 2 ขวบไปแล้ว เนื่องจากเด็กกลุ่มนี้จะมีการเจริญเติบโตทางร่างกายที่ปกติ บางครั้งก็ดูรู้เรื่องและเข้าใจสิ่งต่างๆ ได้ดี เพียงแต่ไม่ค่อย สบตา ไม่สนใจคนรอบข้างและมีปัญหาเรื่องพูดช้ากว่าเด็กวัยเดียวกันแต่อาการข้างต้นจะสังเกตได้ยากในช่วงเด็กเล็ก โรคออทิสติกเป็นโรคที่มีความผิดปกติของสมองซึ่งทำให้มีปัญหาเรื่องการสื่อสารและการสร้างความสัมพันธ์กับคนรอบข้าง มีปัญหาพูดช้าหรือมีพฤติกรรมชอบทำอะไรซ้ำๆ อาการของเด็กออทิสติกแต่ละคนจะมีอาการไม่เหมือนกันความรุนแรงไม่เท่ากัน เด็กออทิสติกที่มีระดับสติปัญญาปกติสามารถสื่อสารได้และพูดได้ตามวัย แต่บางรายที่มีปัญหาสติปัญญาบกพร่องร่วมด้วยก็จะพบว่าพัฒนาการบกพร่องหลายด้านและมีพูดช้าร่วมด้วยได้มาก

นับตั้งแต่ปี พ.ศ. 2550 ผมได้เข้ามาเป็นหัวหน้ากลุ่มที่ปรึกษากรมสุขภาพจิต ภายใต้วิสัยทัศน์ของอธิบดีกรมสุขภาพจิตในขณะนั้น (นายแพทย์ มล.สมชาย จักรพันธุ์) ที่ต้องการให้กลุ่มที่ปรึกษาฯ ทำหน้าที่เป็น “มันสมอง” หรือมือวิชาการให้กับผู้บริหารของกรมสุขภาพจิต หนึ่งในภารกิจสำคัญของกลุ่มที่ปรึกษาฯ คือ การให้ข้อเสนอแนะเชิงนโยบาย (Policy Brief) ให้กับผู้บริหาร โดยมีการประชุมทุกสัปดาห์ วิเคราะห์ทั้งสถานการณ์สุขภาพจิตภายนอก เช่น ภาวะวิกฤติ ข่าวสุขภาพจิตและระบบภายใน โดยเฉพาะที่เกี่ยวข้องกับงานวิชาการต่างๆ หัวใจสำคัญของการจัดทำข้อเสนอแนะคือ การให้ข้อคิดเห็นเป็นทีมหรือองค์คณะที่ประกอบด้วยผู้เชี่ยวชาญ ผู้ทรงคุณวุฒิและที่ปรึกษาอาวุโส โดยมีผู้บริหารกรมฯเป็นประธานการประชุม การพิจารณาจะต้องมีการเตรียมข้อมูลทางวิชาการมานำเสนอ มีการอภิปรายอย่างกว้างขวาง และสรุปเป็นข้อเสนอแนะจากที่ประชุม ก่อนที่จะมอบหมายผู้เขียนเพื่อจะนำเสนอเป็นเอกสารให้อธิบดีพิจารณาและสั่งการต่อไป
ขอบเขตของข้อเสนอแนะในเอกสารนี้ ได้รวบรวมเฉพาะบางเรื่องที่ผมได้รับมอบหมายให้เป็นผู้เขียน โดยแบ่งเป็น 5 หมวด คือ 1) สุขภาพจิตกับภัยธรรมชาติ 2) สุขภาพจิตกับวิกฤติการเมืองและประเทศ 3) สุขภาพจิตจากประเด็นข่าว 4) นโยบายระบบและงานวิชาการสุขภาพจิต และ 5) งานวิชาการต่างประเทศ ทั้งนี้ ไม่รวมถึงภารกิจที่ได้รับมอบหมายภายหลัง คือ การกลั่นกรองข้อเสนอแนะจากการตรวจราชการ ซึ่งเป็นความรับผิดชอบโดยตรงของสำนักยุทธศาสตร์สุขภาพจิต (สยส.)

ศาสตร์ส่งเสริมป้องกันสุขภาพจิต เป็นการรวบรวมงานวิชาการ ทั้งในรูปแบบบทความต้นแบบและการสัมภาษณ์ ในงานส่งเสริมป้องกันด้านสุขภาพจิตของผม ตั้งแต่ยุคเริ่มต้นการทำงานจนกระทั่งเกษียณอายุ โดยจัดลำดับเนื้อหาไปตามช่วงวัย เริ่มจากสมองกับพัฒนาการในแต่ละช่วงวัยในภาพรวม จากนั้นจึงกล่าวถึงการส่งเสริมและป้องกันปัญหาในแต่ละวัย ดังนี้ สมองกับการเรียนรู้ทุกช่วงวัย การส่งเสริมพัฒนาการเด็กปฐมวัย ร่วมกันสร้างพลเมืองเด็ก ปัญหาเด็กวัยเรียน การตั้งครรภ์ในวัยรุ่น วงล้อแห่งสุขภาวะ สุขภาพจิตในสถานประกอบการ ทำความเข้าใจเรื่อง “การฆ่าตัวตาย” ความสุขสูงวัย
